My girlfriends, friend is currently living here, she says they leave the doors unlocked after closing and anyone can enter the building undetected, her friend also had all her clothes stolen by a room mate, since the last time she visited her. The place has gone really downhill since my dad stayed there in 2023,

If I could leave less than one star review I would. My dad was there 12 days — In those 12 days: •He fell twice. 5 minutes after we left first day (they contacted us after) – then fell on 9/16 – and did not notify us and told us it wasn’t our business because we didn’t have POA – which we do and they have known since day one we had – they just didn’t want to be yelled at. When I asked for the incident reports for those – the nurse said she hadn’t finished them still – but supposedly would email them to me. ( An incident report for a patient or resident fall must be completed immediately after ensuring the person’s safety and finishing the immediate clinical assessment, typically before the end of the current work shift – I still have no received them) During care conference they stated that my dad said not to call me – he said that was not true and his roommate corroborated that my dad had asked the nurses to notify me. •His blood sugar was out of control (it was stable when discharged from hospital) and now it’s stable after I forced them to do their actual job. And proved they had been falsifying documentation around taking his CBG. They continued to give him regular diet which caused his sugars to be between 250-380. • His pain meds were withheld from him. – they would turn off his call light and tell him to have patience and walk out. • I brought up my concerns to the director who was condescending and stated “my family ran nursing homes in Idaho I’ve done this work a lot” – yet when you look at his linkedin he was a caregiver less than 8 months. Upon talking to him – he said “if you want me to do something for you concerning your dad you need to do something for me” then said he wanted privacy and proceeded to try and close the door to my dads room while I was in there alone – then seemed annoyed when my dad stopped him because he had came in from smoking right at the second Josh attempted to close me in the room alone with him. • He had no occupational therapy and has only had group physical therapy instead of one on one – and has not focused on any items he needs – like walking, transferring, using restroom alone ect. Yet his discharge paperwork says he was given occupational therapy, physical therapy, and speech therapy. Which he only had PT -and that was far and few between. • Upon signing discharge papers and getting the medication – I looked in the bag and they didn’t include his insulin – thankfully we hadn’t left yet. I had to go remind the nurse to give me his insulin- as she “forgot” he was a diabetic. •I noticed his left leg and ankle was swollen – notified PA. They said they would order compression stockings – all they did was look but did not take his socks off or do any real investigation. Next day I got here and brought him compression stockings – his left leg was hot – and he had a lot of pitting on ankle area. When I lifted his foot I saw this huge sore about the size of a quarter- which was not there before. The bottom of foot is red and hot. I asked what he did to his foot and had no idea what happened. He wears socks at all times and I’ve washed his clothes – none of which had holes in them if he had dragged his foot to open it. He had a callus there prior – and a few days ago the feet people (should have been a nurse since he needs diabetic foot care – but with this place god knows who touched his feet) had messed with his feet. This caused a major infection in which he had to be placed on high dose antibiotics at new facility. I notified the nurse – she said oh it’s healed it’s fine and put a bandaid on it. Then wrote on discharge paper “healed blister” for his foot area. — You will constantly hear people yelling for help – Cnas and nurses ignore call lights – they will go in the rooms and turn them off and walk right back out. They left my dad in briefs because they didn’t want to assist him in using the toilet and then would leave him in soiled briefs over 2.5 hours so he ended up with a fungal infection in his private areas. The first day we were there both the nurse and cna said he was NOT ALLOWED to use the restroom. They refused to help him eat when he was a 1 on 1 feeder when he got there due to paralysis of right arm and weakness in left. My husband and I were there in shifts so one of us would always be there at least during meals because we found out they weren’t feeding him or helping him. The facility is supposed to do full body skin check at admit – they failed to see not one but 2 nicotine patches on him that were old – one was left on two days and one was left on for 6 with a date from when he left hospital. They don’t check who comes in or out – and people can come in at all hours of the day and night without anyone asking who they are or asking them to sign in placed vulnerable people in possible harms way. I reported them to adult protective services, ombudsman, and the state board of nursing. This place is a disgrace and absolutely atrocious- if you care about your loved one – do NOT send them here!

Terrible service, they don’t care. They never ever answer their phones no matter what time of day you call and they hang up on you when you’re on hold. It’s impossible to get ahold of any one there. Doesn’t help that they make a bunch of fake 5 star reviews to try and combat how terrible they actually are

The staff here is the best ! They are truly good to all ages that vary. If you need or would like something, all you have to do is “ASK” !

My Mom had to be taken back to the hospital for lack of care and became septic. Then threw her things away and didn’t bother to call all contact numbers for someone to get them. Most employees do the bar minimum and I pay the hard workers, which there were obviously few, find a better place to work.

I have had the good fortune of being plan in the care of Bridge Crest post accute 2 times in my short 35 years that I have been alive and I honestly lived it! I hated the situation that I was in of course. The way that they handled my ordeal though, made it not only bearable, it was kind of a blast. From the head honcho, to the administration, to the attending physician’s, to the nurses/cna’s, to the maids, to the janitor and the maintenance guy, to the receptionists!!! The crazy thing is that my mother who is currently 65 years young, has also had two stays there! Although that was when it was called Discovery and owned by Prestige. Regardless, it was the same building, same format, and even though it was nearly 7 years ago I would guess that well over 50% of the staff are the same. I can’t speak for my mother or recount all of her experiences while she was a resident/patient. I can, although say that whenever we reminisce together about it she is half laughing and crying because she’s such a softy. I think it says alot about a facility that can allow anybody regardless of age, gender, ethnicity, or background feel just as at home as the next person. I would like to give a shout out to everybody at Bridge Crest that might read this! I love and miss you all!!! I wanted to save the best, most amazing, caring, awesome employee for last so that I can give her a shout out. Jennifer Fidler is currently I think the activities director and I must say she absolutely kills it!!! But if it were up to my mother and myself and I am pretty sure if you asked all of the rest of the residents they would agree that she is truly like the key stone to that whole entire place. If ever they were to lose her as an employee I feel like it would slowly but surely waste away…… I’m being a little dramatic there!!! But she is absolutely the best and we all love her so so much.

Dirty poor communications

Spent over 3.5 months in this facility. Service was ok. Had its ups and downs like most places I’m assuming. I’m only 36, so i was definitely youngest one here and dont have much experience to compare it to. The reason i gave it 4stars, is because the food was not good, and some of the staff dont seem to care much about their patients. But that is only a select few. For the most part though, everyone is very nice and the PT staff is amazing. I had the Privilege to get to work with Nora. She truly cares about making sure you get your work in and you leave here 100% times better than when you showed. Overall. Place is pretty good and will probably be back in a few months after my surgery! Thank you Bridge Crest! Michael

My father came from another facility and this place was a night and day difference. I did a tour, fell in love with the scenery and open concept with lots of light… and I even tried the food which was pretty good. I’m also really really happy they had designated smoking areas for him. Plus everyone has been so friendly to us! I’m glad OHSU recommended this place!

As I am sitting in my dad’s room bawling my eyes out because the incompetence at this place is astounding! His time here has been pure misery (with the exception of a few good nurses) this place cannot get anything right. And now he is discharged, no longer their responsibility and yet sitting in their room waiting on transport. Transport was ordered by them and when it was wheelchair transport and not a stretcher, which was specifically ordered, they said oh well nothing we can do. So what he just sits here?!?!

My experience has been really good the staff have been really helpful with my care and very friendly and confident. They have gotten my diabetic sores down to almost completely gone. My rides to my appointments are always ready and on time. The food is great! I would highly recommend Bridge Crest Post Acute for their after care needs.

The staff and personnel have all been attentive and personable. I have 4 more days here, but I’m sure my review will continue with the 5 star rating.

I was there for 6 months. Most of the aids were very nice. Buy they were so overworked they barely had time for bare necessities. The “nurses” only had time to pass meds. And not much else. Substandard care out side of that. Took about 3 weeks to diagnose my UTI.. then had to go to my urologist. I learned how to do my own Ostomy. They don’t have time to help with that, IF they even know how. The PA’s…I really liked 1. He listened and worked with me. The others were more about pushing pills. I had to sternly tell one how to do his job. As for the food…I lost 30 pounds in 3 months..I could barely eat it was so horrible and I am not that picky. The carpet in the place has to be at least 50 yrs old. It is absolutely disgusting. I think management lies to live ones looking for a place and they don’t care about anything except money. Patients and workers pay the price. I gave 2 stars only because most of the aids did the best they could

I’ve had a lot of fun here the staff are great they’ve taken great care of me and my legs and they’re helping me get my legs back together and get my body back in shape and I feel greatly appreciated . I would recommend Bridgecrest to anyone because they’re a great they got great personality and they’re very upbeat and they have bingo and we play several times a week.
